Vivify delivers remote monitoring for home care patients

Vivify Health is to deliver remote patient monitoring services to Windsor Place At-Home Care's home care patients across Kansas.
The Texas company is a developer of virtual products for holistic patient care and engagement. Windsor, based in Coffeyville, Kansas, has employed remote care management since 2008, but said it switched to Vivify from a large telehealth vendor with international operations because of the better personalisation and efficiency.
"With the help of Vivify, we are able to take care of more people at home without increasing our staff," said Monte Coffman, executive director of Windsor Place. "Vivify's unique risk stratification algorithm improves the personalisation of care, and its fully managed remote monitoring kits make telehealth very simple for both our patients and our staff. We also like Vivify's superb education videos, which teach our patients how to take better care of themselves."
Vivify Health's back-end software enables Windsor's nurse care managers and coordinator teams to support 250 patients each while continuing to deliver good outcomes. Windsor staff received four days of training last summer and already feel comfortable using the Vivify product.

As an additional enhancement, Vivify Health recently finished writing an interface with the Kansas Health Information Network (KHIN), allowing physicians connected with the health information exchange to view biometric monitoring data and nursing notes on the patients they share with Windsor whenever they wish. Through this window, physicians and other caregivers can gain visibility into the condition of their patients between office visits.
Windsor Place At-Home Care is a licensed home health agency with offices in Coffeyville, Independence, Kansas City, Chanute, Columbus, Sublette and Lawrence, Kansas. Its mission is to provide independent living support services and supports for persons so they remain in the comfort and security of their own home. The agency is part of a larger organisation that also includes skilled nursing facilities and assisted living facilities in Kansas.
