Axxess and CHL partner on remote patient monitoring

Texas-based home healthcare technology firm Axxess is helping its clients with the PDGM patient-driven groupings model by partnering with Californian company Connected Home Living (CHL) to provide telehealth and remote care monitoring.

Tantingco said CHL's data show 94% of patients currently using its technology can stay at home without hospital or emergency room visits following an in-patient stay. Among services provided by CHL are clinical monitoring available round the clock, medication monitoring, arranging for transportation, supplies and medications, providing up-to-date educational modules, and the collection of critical clinical data.
